In accordance with Law 25 on the protection of personal information, this form is intended to collect your free and informed consent regarding the collection, storage, consultation, and disclosure of your personal information (PI) through information and communication technologies (ICT) as part of the services offered.

1. Collection, Storage, Consultation, and Disclosure

Your PI (contact information, health details, billing, etc.) is collected, stored, and consulted through secure software to ensure quality care and proper management of your file. It may be shared with internal or external stakeholders (e.g.: doctor, CNESST, SAAQ) when necessary, always in compliance with Law 25. No social networks are used to transmit your PI.

2. Authorized Persons

Only the following persons may access or receive your PI: yourself, clinic staff bound by a confidentiality agreement, police or a competent authority if required by law, your legal guardian (if under 14 years old), and external persons or organizations authorized by you.

3. Risks and Protective Measures

Communications via ICT entail an inherent risk of interception. Évolution Physio Clinics implement technological means to protect your data (username, password, access audits). Only what is strictly necessary is shared.

4. Your Rights

• Access your PI and request corrections if necessary;
• Withdraw your consent at any time (legal or ethical obligations may require minimum retention);
• Be informed in case your information is disclosed to another third party, with your agreement.

5. Retention Period

Your information is kept for 5 years after the last service or purchase, in accordance with our legal obligations.
